We're here to help you

We know that, even if you have been diagnosed with a life-limiting illness, sometimes all you want to do is get on with your life in the best way that you can. We are here to support you to do just that.

It is our aim is to enable you, as far as possible, to lead your life in the way that you would wish. We can help in a variety of ways to support as many people and their families living with a life-limiting condition in our community as we can.

We can also help by signposting patients and carers to other services which could help them.

We see the types of care we can offer as vital in enabling you and your family to live positively with your condition.
