Warrington Palliative Virtual Ward

Sometimes, a patient's care needs are complex, but they still want to be in the comfort of their own home. The Palliative Virtual Ward is a remote monitoring service designed to provide a high level of care for palliative and end-of-life patients, without the need for a hospital or hospice admission.

How it works

The Palliative Virtual Ward is delivered by St. Rocco’s Hospice. We provide short-term support and monitoring of clinical symptoms within your own home. 

You will have an initial face-to-face assessment with a senior nurse. We will then work together with you to develop a plan of care and provide continued monitoring of your symptoms via telephone or the Luscii App. Anyone can access the Palliative Virtual Ward service if they need help with unmet symptoms and palliative care support following a referral from a GP or health professional.

Is the Palliative Virtual Ward right for me?

The Palliative Virtual Ward is for patients who have symptoms that might otherwise lead to an inpatient admission. It's also a great way to be supported at home if you have:

  • Uncontrolled symptoms related to your palliative diagnosis.
  • Had a recent medication change that requires monitoring.
  • Are at the end of your life and wish to stay at home while your symptoms are managed.

Our specialist team works closely with your GP and other multi-professional teams within the hospice and wider community. This ensures you get the right support, making sure your care can be adjusted as your needs change.

Who do you need to tell?

Please advise all medical and health professionals involved in your care that you are receiving care from the Palliative Virtual Ward for us all to work together to optimise your support.

What happens after discharge from the Palliative Virtual Ward?

When you are discharged from the Palliative Virtual Ward, your care will be transferred to either your GP or other healthcare services.
