Palliative Virtual Ward
Sometimes, a patient's care needs are complex, but they still want to be in the comfort of their own home. Our Palliative Virtual Ward is a remote monitoring service designed to provide a high level of care for palliative and end-of-life patients, without the need for a hospital or hospice admission.
How it Works
Once a healthcare professional refers you, we aim to meet you either at home or at the hospice within 24 hours. Our team of Palliative Virtual Ward Nurses will offer different communication options to meet your needs, including phone calls, messaging, or the use of an app called Docobo. This allows us to check in with you regularly and keep a close eye on your symptoms and well-being.
Your care is primarily supported by our small team of nurses, with help and oversight from the hospice medical team. When needed, our team will refer you to other professionals to assist in meeting your needs.
Is the Virtual Ward Right for Me?
The Palliative Virtual Ward is for patients who have symptoms that might otherwise lead to an inpatient admission. It's also a great way to be supported at home if you have:
- Uncontrolled symptoms related to your palliative diagnosis.
- Had a recent medication change that requires monitoring.
- Are at the end of your life and wish to stay at home while your symptoms are managed.
The Palliative Virtual Ward team works closely with your GP and other multi-professional teams within the hospice and wider community. This ensures you get the right support, making sure your care can be adjusted as your needs change.
